Billing / Payment

Payment for services is due at the time of the visit, unless arrangements are made previously. We accept cash, check, Visa, Discover and MasterCard.

If you are a member of a managed healthcare plan, you must present your insurance card and copay at every visit. Your plan requires that we collect a copay at the time of each visit.

Payment must be made by the person bringing the child in for the visit. There are no exceptions.

Returned checks will be charged a $25 service fee.

  • BCBS PPO

  • Cigna HMO & PPO

  • First Health/Coventry

  • Humana -Choice Care PPO only

  • Healthsmart PPO

  • PHCS PPO only

  • United HMO & PPO

**With the continual changes that managed care plans make, it is the policy holder’s responsibility to call their insurance plan and confirm that Albert G. Karam, MD is a contracted provider on their specific plan. If you fail to do so, you may be responsible for the full cost of the office visit, including immunizations and labs.
